When the TonoPen first came out, it was invented by the old company “Mentor,” whom sold the device to “Medtronic,” whom finally sold the technology to the current manufacturer “Reichert.” Since that time, the device has changed very little– even it’s shape and size have stayed pretty much constant. We don’t very much mind I suppose, as the device fits in the hand perfectly, does the job comparatively well to a Goldmann Application device, and avoids the dreaded “puff” altogether. And so therefore it looks like a winner. But it should come with saying that the unit does require numbing drops be administered to the patient– yet another tonometry-related thing that will drive patients crazy. We’ve heard horror stories about patients outright refusing any form of IOP measurement from previous experiences and it’s not hard to see why. If it’s not a puff of air, it’s a drop, and then sometimes it’s all of the above- none of these promote a friendly patient experience. We get that IOP measurements have to be invasive to some extent and we also understand that a pretest room must “flow.” So therefore we offer this hardline: a Puff-NCT is efficient but invasive, and the TonoPen is slightly less invasive but less efficient, and other devices such as the iCare Tonometer aren’t really efficient either and they still freak patients out–despite their claims. Maybe we’re just unsatisfied with forms of Tonometry altogether.
